HFFA MetLife Supplemental Group Benefit Plan
Learn more about these voluntary, supplemental benefits — carefully designed and underwritten by MetLife to provide the highest level of coverage with the best group rates, specifically for our HFFA active-duty members. Premiums are paid conveniently through payroll deduction.
Eligibility: All full-time members actively working a minimum of 30 hours are eligible to enroll during open enrollment. Documents in the member portal are for members only.

Accident Insurance
Helps manage medical costs from accidental injuries on and off the job. Benefits are paid directly to you for initial care, injuries, and follow-up — in addition to any other coverage.
Hospital Indemnity

Participating Hawaii attorneys can assist with wills and trusts, family law, traffic, and more, organized by legal topic.
Critical Illness w/ Cancer
Coverage that pays a benefit on diagnosis of a covered critical illness, including cancer.
Portable at Retirement
Members who signed up while active duty can convert their plans to portable coverage. Ask about the details.
🎁 Get $100 Health-Screening Credits
The Critical Illness w/Cancer, 24-Hour Accident, and Hospital Indemnity plans include a $100 health-screening credit for the employee and covered family members. See the eligible-screenings list, then submit the Health Screening Benefit Form. Register first on the MetLife MyBenefits page.

Portals & Claims
MetLife plan participants should register on both portals. File claims at the MetLife MyBenefits page (group #231849), and manage enrollment through the PEC enrollment portal.

Dental Coverage
Dental insurance is not included in the MetLife plan. For dental, contact EUTF — Oahu (808) 586-7390, Toll-Free 1-800-295-0089. Note: if you carry the 24-Hour Accident plan, you are covered for certain dental repairs resulting from an accident (see your Accident Certificate).
